How to get KY Death Certificates, 1911 to 1956

FROM 1911 to 1956.
If you send an order outside of these years, you
may not get your money back.
Many people have asked how to
find Death Certificates for Kentucky on the web.
At the bottom of this
page is a link to the Rootsweb Ky Death Index Database.
After
locating the Death Certificate you are looking for you can order a copy
of
that certificate from: KY Vital Statistics, 275 East Main, Frankfort KY 40601
The cost is $6.00 each......OR
If you have someone who died
between ****1911 to 1956*****,
I have access to these certificates on
microfilm. I would be glad to make
copies of these certificates for $3.00
each. I will usually make
copies on Tuesdays and Thursdays, so certificates
are almost always mailed in
three days of receiving an order.
If you would like copies of any
certificates, please send $3.00 (3.50 if you do not supply Death Volume and
Certificate Number)for each
certificate with a self addressed
stamped envelope to the address below. Multiple requests may be sent in one envelope with one check and one SASE (please include enough postage).
Please send the name and age of the
deceased, the county of death, and the
date of death. If you have already
searched the index please send the certificate number and death volume.
If you do not have
the certificate number and Death Volume, there is an additional charge of $ .50 if I have to
search the index. If I cannot find your
person, the $3.50 fee is not refundable.
These
are not certified copies on the Blue and White Kentucky State Paper. These
are black and white
copies from the microfilm copy of the original
certificate.
