Hugh Thomas Crouch (MD) 5 ( Richard J. 4, Richard 3, John 2, Richard1) was born September 10, 1856 in Ballard County, KY; he was favored in youth with a good English education; he died 1936 in Bardwell, Carlisle Co, KY; married on May 27, 1884 to Mary U. Willingham daughter of George Wand and Esther A. (Peck) Willingham; she was born October 13 th,1864; she died September 2, 1920 at age 55 years in Bardwell, Carlisle Co, KY.
In 1879 Hugh Thomas commenced the study of medicine with Dr. DP. Jouett of Blandville, Ky and in 1880 attended lectures at the Missouri Medical College, St. Louis, Mo., where he graduated in 1882. Initially he engaged in the practice of medicine in his native Ballard County, but latter moved to Bardwell, Carlisle Co., Ky. In February of 1909 he was President of the Board of Health of Carlisle County and enacted a quarantine ordinance that prevented any people arriving by train from the City of Cairo, Illinois or Mounds City, Illinois from alighting into Carlisle County due to the smallpox epidemic raging in those two citys.[4]

3. Richard Osler Crouch 6 born about 1896, Kentucky; died 1963 with burial: Old Bardwell Cemetery [2] He married Marriage 1 Rama L WRAY born 1893; she died 1941.

Dr. Hugh T. Crouch, a rising young physician with a bright future before him, was born September 10, 1856, in Ballard County, KY, where he has always resided. His father, Richard J. Crouch, a native of Christian County, KY, came to Ballard County in 1842 and died in 1870 aged about 48 years. Richard J. espoused Martha, daughter of Hugh MASSIE of Christian Co KY, and to them were born Martha E. (Wray), Richard B., Mary E., Susie F., and William H., as well as our subject. Subject was favored in youth with a good English education. He was married May 27, 1884, to Miss Mollie U. Willingham, daughter of George W. and Esther A. (Peck) Willingham of Ballard County, KY. She was born October 17, 1864. In 1870 subject commenced the study of medicine with Dr. D P Juett of Blandville, KY, and in 1880 attended lectures at the Missouri Medical College, St Louis, where he graduated in 1882. He is now successfully engaged in the practice of his chosen profession in his native county.(note - the above lists daughter Mary E. while other sources list her as Mollie. I believe Mary E.'s nickname was Mollie)
