25 Feb 1888 Claim: Henry W Faith E 81 Ind Vols -- Henry W Faith enlisted 28 July 1862, as Pvt, Co E, 81 Regt Ind; discharged at Nashville TN 13 June 1865. ...was disabled by malarial poisoning, resulting chronic diarrhoea from exposure, impure water & improper diet 
at Nashville TN 25 Dec 1862; was treated at Convalescent Camp - Nashville TN.
-----War Dept, Adjutant General Report 27 March 1888 -- Henry W Faith, Private, Co E, 81st Regt Indiana Volunteers, was enrolled 28 July 1862 at Corydon, Indiana (3 yrs) and is reported: on roll from enrollment to Oct 31, 1862, present; Nov & Dec 1862, 
absent - sick in Hospital at Nashville, 26 Dec 1862, same to 28 Feb 1863; March and April 1863, present - same to August 31, 1864; Sept & Oct 1864, not on file. Returns for Sept, Oct, and Nov 1864, do not report him absent. Roll for Nov & Dec 1864, 
present; Jan & Feb 1865 (last on file) absent Detached as Teamster in Div. Supply Train since 15 Jan 1865. Returns for March, April and May 1865, absent in supply train since 15 Jan 1865. Mustered out with company 13 June 1865, at Camp Harker TN. 
Company morning report March 22, 1863, "Returned from Hospital at Nashville March 22, 1863." Regimental Hospital records with William D Fouts, Jeffersonville, Ind. Records of this office furnish no further evidence of disability.

-----Affidavit 5 Feb 1901 Harrison Co IN; RE: Increase Pension Claim #416071 - of Henry W Faith, who was a Private Co E 81st Regt Indiana Volunteer Infantry. Personally appeared before me a Notary Public in and for said county and state aforesaid 
Mrs. Malinda A Faith, who being duly sworn upon her oath says that she is the lawful wife of said soldier, Henry W Faith, who is a pensioner of the United State as above stated said Pension being for disability of Chronic Diarrhoea and resulting disease 
of Rectum, that her said husband is at this time and has been for the past five months suffering with his said disabilities and the resulting nervous debility and paryalisis of his body and limbs, that he has gradually grown worse and more helpless to such 
an extent that she has had to take constant care and attention of him having to put on and take off and button and un button his clothes - and to assist him in and out of his bed, besides having to feed him like a child he being so weak and helpless. 
Affiant further says that he is so disabled as not to be able to present himself to a board of Surgeons for examination and requests that the Commissioner may order a Surgeon to make the examination at his residence; at Mauckport, Harrison County, Indiana.

before me M A Wolfe a Notary Public.... Henry W Faith aged about 62 years, a resident of Mauckport... who being duly sworn...declares that he is a Pensioner of the United States, duly enrolled at the Indianapolis Indiana Pension agency at the 
rate of ($12.00) Twelve Dollars per month, under Pension Certificate No 416071, by reason of disability from Chronic Diarrhoea and resulting disease of Rectum, incured in the Military Service of the United States, in the War of the Rebellion, 
while a Private in Co E of the 81st Regiment Indiana Volunteer Infantry, that he believes himself to be entitled to an increase of pension on the account of his rate now being unjustly low and disproportionate to his degree of disability and their 
results and in addition to above disabilities as a result, he is now and has been for the past five months, suffering from resulting nervous Debility, and which has caused partial Paralysis of his arms, body, and legs which is bordering on 
Locomotor (attapia?), to such an extent that he has but little us of himself, and most of the time he cannot write his name, or dress or undress himself; that his condition is such as to demand the constant care and attention of his wife or some 
other person; that he is most of his time confined to his house or bed; that he believes his said resulting disabilities to be caused from and the results of his said disabilities for which he is pensioned. He further says that his said disabilities are
 not the result of visious or intemperate habits, that he requests that a special examining Surgeon, be at early day sent to his home, here at Mauckport, Harrison Co IN, to examine him, as his physical condition is such that he could not leave

Mauckport IN 20 Feb 1901 State of Kentucky, Meade County In the matter of increase Pension Claim #416071 of Henry W Faith who was a private Co E 81st Regt Indiana Volunteer Infantry -- personally came before me a Circuit Clerk 
in and for said county and state aforesaid, Dr. John D. Burch, aged abt. 50 years, whose Residence & PO is Brandenburg, Meade Co KY, being duly sworn, declares: I am now and have been for the past 24 years been regularly engaged 
in the practice of Medicine. I have know said Henry W Faith about 1 1/2 years; at time of my first acquaintance I was called to see and give him medical treatment, at said time I found him suffering with Chronic Diarrhoea and disease of 
Rectum, which diseases as I was informed were acquired in said service; he was also suffering with resulting Nervous Debility. I have given him medical treatment at intervals since my first treatment. I find as a result of his said disabilities 
he is now and has been for the past five months, suffering from said resulting Nervous debility, and which has caused partial Paralysis of his body, and limbs, which is bordering on Locomotor ataxia, to such an extent that he has but little 
use of himself. His condition is such as to demand the constant attention of his wife or some other person, and he is confined to his room or bed the most of his time. I consider his disabilities permanent, and on the increase - do not think 
he would be able to leave his home to attend a board of examining Surgeons. I think his condition such that he is entitled to be examed at his home. I have no interest in this claim. John D Burch, M. D.

-----Declaration for Orignial Pension of a Widow 27 Nov 1901
...Personally appeared before me, a Notary Public, Malinda A Faith, aged about 60 years, a resident of the Town of Mauckport in Heth Township, Harrison County, State of Indiana, who, being duly sworn according to law, makes the 
following declaration in order to obtain the pension provided by Act of Congress granting pensions to widows, approved July 14, 1862, and supplemental acts amendatory thereof, to wit: That she is the widow of Henry W. Faith, deceased, 
who was a pension of the United States, by Certificate No. 416071, who served during the late War of the Rebellion under the name of Henry W. Faith, from the 28th day of July 1862, to the 13th day of June 1865, as a Private in company E, 
81st Regiment Indiana Infantry Volunteers, and who was Honorably Discharged from the service on the 13th day of June, 1865, at Camp Harker, State of Tenn, and who died of Chronic Diarrhoea and resulting disease of Rectum, on the 18th 
day of November 1901, at his home in Mauckport Ind., that she was married under the name of Malinda Deweese, the letter "A" of her middle name having been from oversight omitted in their marriage license, to said Henry W Faith, who was 
licensed under the name of Henry Faith, the letter "W" of his middle name also was left out in their marriage license, through the mistake or oversight of the one who procured their license, which will fully appear by refering to a certified copy of 
same which is hereto attached and made a part hereof, that they were married on the 9th day of August 1857, by Hiram Darter, who was a Justice of the Peace in Heth Township said county and state, at her home near Mauckport Indiana their
 being no legal barrier to such marriage; that said Hiram Darter is now dead, that neither she nor her husband had been previously married nor ever divorced; that she has not remarried since the death of the said Henry W Faith; that he left no 
children under the age of sixteen years; that she has not in any manner been engaged in, aiding or abetted the rebellion in the United States; that she has not heretofore applied for pension; that since the 13th day of June 1865, the above named 
soldier was not in either the military or naval service of the United States; that she files her said claim and has no attorney. That her residence and post office address is the Town of Mauckport, Harrison County in the state of Indiana.

...on 27 Nov 1901 personally appeared Malinda A Faith....declares she is the lawful widow of Henry W Faith, deceased; that he d 18 Nov 1901; that he had been granted a pension by cert #416071, attached; that he had been paid 
the pension by the pension agent at Indianapolis Indiana up to the 4th day of November 1901 after which date he had not been employed or paid in the Army, Navy, or Marine service of the United Sates; that she was married to the 
said Henry W Faith on the 9th day of August 1857, at her home near Mauckport Harrison Co IN; that her name before said marriage was Malinda A Deweese; that she had not been previously married; that her husband has not been 
previously married; that she hereby makes application for the pension which had accrued on aforesaid certificate to the date of death; and that her residence and post office address is in the Town of Mauckport, Harrison Co IN.

Sir: Hereto attached please find my application for Widows Pension - also application for accrued Pension that is due me from 4 Nov 1901 to 18 Nov 1901 it being 14 days - at $12.00 per month from Nov 4, the date of last payment 
to my husband Henry W Faith - who died on the 18th day of November 1901. Also find his pension certificate No 416071 which is herwith returned. Also find a Certificate from the County Clerk of License record showing date and
 place of our marriage. Also affidavit from James Faith, who was an eye witness to our marriage, also affidavit of Dr. William R. Denbo who was the attending physician on my said husband at time of his last illness and death, and who, 
is also the Health Officer for the Corporation of Mauckport, Indiana, also affidavit of Emmette M. Beanblossom, who is Heth Township Trustee in Harrison County Indiana; and also was the Undertaker who furnished the Burial supplies 
and burried my said husband. Also the Affidavit of Hugh B Blake, who is an old acquaintance and neighbor; and also affidavit of George M Miller, an old neighbor; that I wish you to consider and accept. Said Hugh B Blake and George 
M Miller's evidence in proof of our marriage, with that of James Faith. Said James Faith, being now the only eye witness to our marriage that I after diligent inquery could obtain; also please accept all the evidence herewith sent in lieu of 
my identification, etc, please acknowledge the receipt of above evidence and give my claims special attention as soon as possible, as I am in destitute circumstances, also inform me if the evidence etc, herewith attached and sent the 
department is sufficient to establish my said claims for Widows Pension and Accrued Pension. Repectfully, Malinda A Faith.
